RN Care Manager

A private family in Indianapolis is seeking an experienced RN Care Manager to provide comprehensive care management and support for elderly parents in their residence. This is an excellent opportunity for a seasoned RN with geriatric and dementia-care experience who enjoys working directly with families and can bring structure, sound judgment, and continuity to an evolving care environment.
The successful candidate will take the lead in coordinating care, monitoring health and medications, communicating with medical providers and family members, and managing additional caregivers as needs evolve. This individual should be patient and compassionate, with the ability to build trust while maintaining appropriate boundaries and providing the family with clear, consistent communication.
Responsibilities:
- Oversee and coordinate day-to-day care
- Monitor medications and maintain medication inventory
- Monitor physical and cognitive changes, food and fluid intake, and overall well-being
- Coordinate medical appointments and communicate with healthcare providers
- Develop and maintain care plans, schedules, and records
- Hire, schedule, and supervise additional caregivers as needed
- Assess the residence for safety and recommend appropriate modifications
- Provide occasional transportation for medical appointments
- Serve as the primary care contact and respond to emergencies as needed
- Collaborate with family members, the family office, and household staff
Qualifications:
- Active RN license
- Strong geriatric and dementia-care experience
- Minimum 10 years of nursing experience preferred
- Prior nursing leadership or supervisory experience preferred
- Excellent communication and organizational skills
- Patient, compassionate, and skilled at building trust
- Professional presentation, discretion, and sound judgment
- Valid driver’s license
- Must live within a reasonable commute of the Indianapolis (Southwest) residence
Schedule & Compensation:
This is a full-time salaried position, approximately 40–48 hours per week, requiring flexibility for emergencies. Compensation is $90,000–$120,000, commensurate with experience. Benefits and a company cell phone are provided. A sign-on bonus may be considered.
